Factor out render setup

Encapsulates special handling of Arthur vs. Splash in new classes in
Qt5OutputDevices.cc. Shifting this stuff away makes the code reusable from
outside poppler-page.cc, and it leaves the logic regarding Page::renderToImage
clearer.
