I downloaded the original attachment and filled it out using Adobe Reader DC
for Windows 7, then saved it. This is the result.
It's worth noting that Adobe did not recognize any values existing in the
original file when I opened it, and also that the filled-out version (attached)
renders correctly with poppler.</pre>
