[poppler] More about annotation improving

I have continued a little with my ideas. As didn't have time to split it
on more patches neither document it better as changes are becoming
bigger, but I think how the idea works can be see easier.

I still need to see how stream works, the form/xobject implementation as
appearance streams needs it and see how gfx context is build to draw annots.

Here goes the new patch.

Any question/suggestion is welcome.
