After reading your reply, I found the following in PDF32000_2008.pdf
(page 388):

  The normal appearance shall be used when the annotation is not
  interacting with the user. This appearance is also used for
  printing the annotation.

There is another issue with the border. Why is the stroke width constant?

I mean, if the stroke width is measured in user units, it should be
proportional as the annotation border itself, shouldn’t it?
