 pulseaudio can successfully play audio over my ALSA driven USB headset.
 However, when the volume is changed in pavucontrol, the audio crackles
 then the sound disappears.  This log entry appears at the same time:

 pulseaudio[26081]: module-alsa-sink.c: failed to get delay: Broken pipe

 This is what the kernel dumps into the log when I plug the USB headset in:

 kernel: usb 4-1: new full speed USB device using ohci_hcd and address 4
 kernel: usb 4-1: configuration #1 chosen from 1 choice
 kernel: input: Plantronics Plantronics Headset as /class/input/input7
 kernel: input: USB HID v1.00 Device [Plantronics Plantronics Headset] on
 usb-0000:00:0f.0-1

 Restarting the pulseaudio daemon restores the sound.
