 Yeah, I know, don't remove the think if it's being used, but I
 accidentally yanked the cable when I stood up...

 The kernel reported:

 kernel: ALSA /var/tmp/bach-build/BUILD/alsa-
 driver-1.0.14/usb/usbaudio.c:866: cannot submit datapipe for urb 0, error
 -19: no device
 last message repeated 1022 times

 And pulseaudio crashed.  I would like to see it handle the situation a bit
 nicer -- like switching all the affected streams to other sources/sinks.
