 On closer inspection, I find that compiz's "audible bell" option was
 enabled.  Disabling this resolves the problem as originally reported.

 I still don't understand why the problem only appears initially, and
 disappears when `module-x11-bell` is unloaded and reloaded.  That still
 smells like some sort of race condition to me.  But if you think the
 compiz interaction is too obscure to be worth investigating further, I
 don't mind considering this bug closed.
