 Wasn't sure how much to attach, so I just put everything that showed up in
 my terminal. The interesting bit is at the bottom. And yes, I know I
 wasn't clear, but I have steps to reproduce the problem.

 1. Play something (anything) w/ Simultaneous Ouput as enabled & defaulted.
 2. Stop playback.
 3. Unplug iMic (or other external usb sound card)
 4. Try to play again, PA crashes.
