 This sounds like a bug when pulse is started with a muted device. It
 detects this as unmuted and requires two presses in pavucontrol of the
 mute button to get it back.

 There are redhat and mandriva bugs for this, and I suspect that something
 is causing the device to become muted and it's not coming back up right.
 That said it could be a totally separate bug.
