 Most likely PA is dying when this happens. Please set autospawn=no in
 ~/.pulse/client.conf, then stop the running PA instance with "pulseaudio
 -k". Then run "pulseaudio -vvvv" and reproduce the issue. This should show
 you the reason why PA terminates itself. Please attach the end of the
 output of PA here.
