#913: PA can't change playback volume with Logitech Premium Notebook Headset

I have a Logitech "Premium Notebook Headset" (USB id 046d:0a01) and
PulseAudio is unable to control its playback volume. The device has only
three volume controls in alsamixer: Speaker (Playback), Mic (Playback),
and Mic (Capture). When I try to change the playback volume with the
sliders in pavucontrol, nothing changes in alsamixer whatsoever. This is
the case regardless of which Port I choose in the Output Devices tab or
which Profile I choose in the Configuration tab. Of course changing the
volume directly in alsamixer works fine.

Also, if I change the Port from Analog Speakers to Analog Output then the
Speaker volume in alsamixer drops to 00, and changing the Port back to
Analog Speakers does not restore the previous value. This is especially
bad because a user that is fiddling with the settings in PA may
inadvertently change the volume to zero and will be unable to change it to
 non-zero without using alsamixer.

(Note: PA _can_ control the capture volume just fine.)

I'm using Ubuntu 10.04 64-bit with the standard packages:

 pulseaudio: 1:0.9.22~0.9.21+stable-queue-32-g8478-0ubuntu14
 libasound2: 1.0.22-0ubuntu7

alamixer reports the device's Card as "Logitech USB Headset" and Chip as
"USB Mixer". /proc/asound/cards shows it as:

  2 [Headset ]: USB-Audio - Logitech USB Headset
                       Logitech Logitech USB Headset at
 usb-0000:00:0b.1-2.1.4.4, full speed

As mentioned above the USB id is 046d:0a01. The product packaging calls it
Logitech Premium Notebook Headset and it looks like this:
