I basically have a generic Dell box with integrated ICH7 audio

 0 [ICH7           ]: ICH4 - Intel ICH7
                      Intel ICH7 with AD1981B at irq 23


When I plug my headphones into the front, in alsamixer only the Headphone
slider has an effect, Master does nothing to change volume.

Now Pulseaudio somehow merges these (even if flatvol is off) and the whole
range of Headphone is compressed into the low range and it gets painful very
quickly.

No idea if this is ALSA bug or not. Please advise and forward accordingly.
