I have recently upgraded to Kubuntu 12.04 and noticed this bug. After I log in,
the volume for my internal audio card is set to 100% for ouput (speakers) and
0% to input (mic).

Only the internal card is affected. I have an USB audio card, and the system
remembers what its volume settings were.

I have tried pulseaudio -k and the settings were not reset when PulseAudio
restarted. They only reset after logging in. I have alsamixer open in a VT, so
I can see when this change happens.

I also had a problem when the output method switches to speakers after a
relogin, even if the headphones were plugged in. The problem kind of solved
itself after I disabled my sound card and then reenabled it, but it could still
be related to the one described above.
