When _hotplugging_ a Bose (Companion 5) USB audio device, pulseaudio fails to
configure it. It (seems to) work fine at the Alsa layer, and doing a cold plug
(i.e., plug the USB before booting, or just stopping the pulseaudio server,
plugging the device, restarting the server) works.
This is with Linux 3.5 and pulseaudio 2.1 (Ubuntu 12.10 beta) but it also
happened with pulseaudio 1.3 and 2.0. And this problem was already noted 3
years ago:

Note that the Bose device has a bit peculiar sound profile, as it only supports
4.1, 5.0, 5.1 channels (no 2 channels).
