Improved patch

Here is an updated patch that I hope addresses the issues in Comment #32

All of the update_requested_latency logic is removed and essentially replaced
by update_latency_range.

I did SINK_MESSAGE_UPDATE_MAX_REQUEST and SINK_MESSAGE_UPDATE_LATENCY_RANGE
from the disable_output. Doing this from sink_input_detach_cb() would not work
because the output is still in the list of outputs at that stage.
