I have 2 digital devices, GK104 HDMI and an M5A99FX this happens to both,
therefor the problem would be PA, logically.  Alsa modules are compiled into
kernel, i.e. they are not modules.    

I have flat volumes disabled.  


Case #1: 

ALC892 Digital HDA ATI SB

In log.txt you'll find pluginloader.exe audio stream #1 (wine pipelight) is
playing sound at 100%.  I start a new stream from TESV.exe, SkyrimLauncher.exe
(wine) unhooks AND THE SOUND GOES CRAZILLY LOUD.  decreasing volume on
pluginloader.exe audio stream #1 does not work, audio is same level of loudness
until it hits 0%, putting the level back to 1% causes the audio to come on
CRAZY LOUD level again.  

Case #2: 

HDA NVidia GK104 HDMI

No log, same situation, stream from vlc using pulseaudio, in kde I set gk104
HDMI as preferred over the mobo audio.  While vlc is streaming I launch chrome
and start a native pepperflash youtube video.  Same problem as above.
