There were two output streams open:

1) An HTML5 YouTube video in Chrome
(https://gopro.com/channel/video-of-the-day/frozen-kitten-lives).

2) An echo-cancelled output stream open by Zoiper (open but not active).

One input stream open:

1) A microphone stream opened by Zoiper.

The problem does not happen all the time. It seems to only happen the first
time I try it after restarting the computer. Every time after that (so far) it
has been fine. Strange!
