This sounds like a problem with synchronizing hardware and software volume
changes when using flat volumes. This is a problem that can't be solved
perfectly. You can try to mitigate it by tweaking the
deferred-volume-safety-margin-usec and deferred-volume-extra-delay-usec
variables in /etc/pulse/daemon.conf. Or if you are not a fan of the flat volume
feature, an easier fix is to set flat-volumes=no in daemon.conf.
