Sounds like something pretty nasty going on.

I did notice some crashes in the past using 0.9.10 and usb devices and 
removal etc. but these were usually related to module-combine being used 
and I didn't have time to debug properly... I'll maybe see if I can try 
such things again with the latest pulse and make sure the problems are 
not still present...

If you have module-combine loaded (see paprefs and the "Symultaneous 
Ouptut" tab - make sure the option is *unticked*. Not sure it will be 
what's wrong... I'm just trying to remember where I had problems in the 
past.
