[pulseaudio-discuss] preliminary PA support in RtAudio

umm, folks, I sort of added async PA support to RtAudio. I'm 100% sure
that at least 50% of my code is wrong, so I'd be really grateful if
someone could do at least a basic peer review and point out most
glaring of my errors. <-- it's here

Some parts like abortStream() are not finished at all yet, but is WIP
and I'll update when I have it done.
(Build with __RTAUDIO_DEBUG__ DEBUG_LEVEL=3 for full debugging printouts).
