Hey all,
I fixed up module-profile-switcher, and it should be good to go in.
Changes since the last time:

* We now switch to A2DP for all non-phone streams (as opposed to only
  music streams in my last patch)

* Made sure we don't leave HSP/HFP mode if there is an active
  source-output

* Removed the sink-input state change hook -- don't know why I had it
  in the first place

We can likely merge this with Frederic's module-bluetooth-policy when it
lands, but since that also depends on a fixed-up version of BlueZ for a
crash, I don't want to block merging this on that.
