Changes in v2:
 - Split the patch into four parts.
 - Simplified buffer allocation in pacat by taking advantage of
   realloc's behavior when the input pointer is null.
 - Made the silence buffer in pacat a global variable to reduce
   repeated allocation and freeing of the buffer.
