it looks to me like this is a bug in the KDE version that landed on
Debian not in pulseaudio. The problem may be wispread but undetected,
the default desktop environment in Debian is Gnome rather than KDE, and
most people don't use an external soundcard so they don't need to change
the default soundcard. Strangely I have this problem on a fresh install
of Jessie, while on another install of Debian testing that was
continuously updated in the last year in never popped up.
I'll try to open a bug report on the Debian bug tracker of the relevant
KDE component (kmix or Phonon I guess).
