We use the _t suffix for simple typedef'd types so this can be
pa_volume_ramp (as can pa_volume_ramp_int).

I'm wondering it makes sense for pa_cvolume_ramp to have one copy of the
ramp type for every channel. Do we ever want to support ramping
different channels differently? Or is it just for the convenience of
embedding an array of pa_volume_ramp_ts?

Just a comment (can be done later), but it would be nice to have common
code like this and the version with mapping in a single function
controlled by a bool variable, so we avoid code repetition.

This is a bit awkward -- either the units should be time (in us, so the
API is consistent), or they should be in samples and you need to keep
the rate somewhere so the context of that length is not lost (what if
the rate changes during the course of a ramp?).

All this seems a bit repetitive. Easier to do something like:

This structure needs to be documented so it is easier to understand what
state all of this represents.
