sgml -> txt conversions

BTW, I noticed that a number of non-ASCII characters had been lost
from the text versions of the sgml-based docs that had been checked into the
tree, mostly mangling a variety of European names in contributor lists and
dropping a few registered trademark symbols and multiplication symbols.
They reappeared when I explicitly set my locale to en_US.ISO8859-1 on Solaris
and re-ran the doctools - they wouldn't show up if I didn't set that and I couldn't
get them to work in UTF-8 at all. It appears they've been using ISO8859-1 so
far so I didn't fight too much - perhaps for the next release we can work on
converting to UTF-8. For now, I've rechecked in ISO8859-1 versions of the ones
I saw that lost characters.
