french canadian fix

Attached are several git formatted patches against master which correct various
issues in the kbd-model-map table for translating between xkb and console
keyboard maps.

0001-French-Canadian-xlayout-is-just-ca-not-ca-fr-any-mor.patch - the xkb
layout for French Canadian is now just 'ca', not 'ca(fr)'.

0002-add-Belarussian-mapping-simple-by-and-by.patch - Belarussian
(Byelorussian) entry was missing, this is straightforward, 'by' / 'by,us' like
the other Russic layouts.

0003-add-Hebrew-Israel-simple-il-il.patch - simple entry for Israeli (Hebrew).

0004-add-Kazakh-keyboard-mapping-kazakh-kz.patch - simple entry for Kazakh
(kazakh / kz,us), with switching like other Russic ones.

0005-add-Lithuanian-keyboard-mapping-lt-lt.patch - simple entry for Lithuanian.

0006-correct-Macedonian-keyboard-mapping-X-layout-is-mk-n.patch - xkb layout
for Macedonian is now 'mk', not 'mkd'.

This should somewhat ameliorate the situation described in downstream bug
https://bugzilla.redhat.com/show_bug.cgi?id=889562 , though many many cases
remain, where there simply is no console layout that matches a given X layout.
We could get fancy and start guessing that Georgians probably want a Russian
layout, or something, but I think that way madness lies, we just need to get
xkb layouts working on console outside of Debianland. This is just a stopgap
for Fedora 18.
