If you have a generic icon for a document, and a generic icon for an  
app, you can put them side-by-side with an arrow between them. At  
least, that's the dull, less-than-ideal answer... like I said, it's  
hard to come up with really good ideas for things like this, so  
sometimes you just gotta punt.
