I've updated

 to address Simon's concerns.  Note that this adds a ServiceBusy error for the
case of overlapping SendTones calls.

I've also created a separate spec that keeps StartTone, and implements
MultipleTones (without Pause/Duration arguments; it is assumed that the CM will
define those):
