The way GSM (and oFono's API exposes this) handles this is with rules for the
following scenarios:

 - Unconditional
 - Busy
 - No Reply
 - Not Reachable (ie, phone turned off)

There's also a timeout attached to NoReply.  Multiple rules can be set at the
same time, with some overriding others (ie, if NoReply is set to forward to
555-555-5555, and then Unconditional is set to forward to 333-333-3333, any
calls will immediately be forwarded to 333-333-3333).

The following is supported by GSM and is for toggling multiple rules at once,
but is not exposed by oFono:
 - All Call Forwarding
 - All Conditional Call Forwarding
