In every place where gnome-terminal sets the cursor to left_ptr (the standard
mouse pointer), it is visibly upscaled from scale=1 on a scale=2 output.

This doesn’t happen when the cursor is set to xterm (the I-beam), which is
correctly drawn at scale=2, nor does it happen in Nautilus at all.
