[PATCH fullscreen-shell v6 00/19] Add a fullscreen shell protocol

This is version 6 of my wl_fullscreen_shell patchset.  Version 5 can be

Changes since version 5:

 * Rebased on weston master.

 * Various bugfixes

 * Pulled in Hardening's --no-clients-resize patch
