I'd like to see a paragraph that adds the restriction that <enumref>
inside <bitcase> can only refer to an enum's <bit> members, and
<enumref> inside <case> can only refer to an enum's <value> members.

I know this wasn't true before your 3/3 patch, but I'd like it to remain
true in the future.
