Sound naming spec: problems with prefixes

What sounds can you choose from? Any file from the filesystem (ugly)
or just sounds from the selected theme (limited functionality)?

Does the sound theme automatically change to "custom"? I think it
should work in a similar way to the desktop themes (or be part of it)
not to cause further confusion.
