I get this bug in Linux 2.4.28 with xterm -lc -e mutt.
It blocks while mutt is doing a ioctl(1,TCSETSW,) after it has written some bytes.
The proposed patch for bug #1400 using pipes to synchronize the pty
initialization fixes this one as well.
