[PATCH sgml-doctools] Add support for PDF bookmarks in the xorg stylesheet

Slowly but surely we're cleaning up the documentation.  :)

One of the next step is getting olinks for cross-document linking to
work with xmlto.  I think it's dying because of xmlto's tempdir.
