[RFC] in/out parameter expansion

Adding a BusRec * to in/out can help make the X arch code a bit simpler, 
and is especially nice for dealing with things like PCI domains (the 
Altix code would have been much simpler with this change in place, for 
example, and Ben wants it for PPC stuff).

This patch just fixes up all the core xfree86 stuff, and doesn't address 
drivers or arch code simplification, but I thought I'd post it for 
review/comments/flames.
