Fix to bug #10645: Proper XV pipe selection for output

This patch series fixes bug #10645 by the proper selection of the
output pipe to use for XV output, as well as proper calculation of
destination coordinates.

This patch only fixes the XV pipeline for the i830 and above (I only
patched i830_video.c and did not check i810_video.c). However, I
believe the patch should be applicable to i810_video.c with minimal
change.
