Just to add to the pile:

I ran into this "accessibility" feature in KDE4 while testing it
recently. I remember a dialog box flashing up on screen but it
disappeared very quickly... the reason was I was trying to think how to
start my sentence and subsequently did so while holding the shift key as
I knew it would be a capital letter. In the end I opted for an extra
carriage return and released shift and hit return, just at precisely the
same time as the dialog box almost inperceptivly flashed on to screen.

Took me ages to realise what was up, especially considering the fact
that the setting survived reboots and everything!

So as a general plea with such dialog boxes, please make them modal and
please do not have any default option focused... the user should be
forced to decide yes or no in these cases, not just hit return without
reading them.

(I've moaned to various KDE people already!)
