kms exposes 3 connectors (DVI-0, DVI-1 and DIN (tv)); ums only exposes
2 by default in your case (DVI-0 and DVI-1).  By default zaphod mode
selects two of those connectors since it has no way of knowing which
ones you want to use.  You need to add the following option to the
device sections for your radeon card:

Option "ZaphodHeads" "DVI-0"
and
Option "ZaphodHeads" "DVI-1"

That will assign one DVI port to each instance of the driver in your
zaphod setup.
