fresh install Kubuntu 14.04 (15.10 same problem) XServer never starts

Try this:
sudo apt-get update

Then:
sudo apt-get -s --reinstall install xorg* xserver* plasma-desktop \
kubuntu-desktop

to do a dry run of what will happen, and if it looks good, remove the
-s from the command and re-run it.

It's a massive hammer, but it will reinstall everything related to X
and plasma/kubuntu.

If there's errors after it installs some stuff, run:

sudo apt-get -f install

and it should figure it out. You may need to do that a couple of times.
