Number 1: Move the file selector box to the top.
2: Do we need the user to specify the file type? Can this not be detected
automatically? (Bugzilla does it)
3: Could step 4 in the form be possibly replaced with an explanation that this
can be done through the image editor? I think people who are likely to specify
additional metadata are already likely to know they can do it in the editor, and
if not they will be willing to check it out.
4: The keywords bit could be more slick. The keywords list *could* be displayed
in the input boxes using a Google Suggest-like JS interface. I'm not sure how
complicated this would be to do though! I found this:

well known "ajax" (I feel dirty for saying that) libraries:

and script.aculo.us is great: http://script.aculo.us/demos/ajax/autocompleter
That demo uses Rails though -- don't know how simple it is to do with
PHP/Perl/whatever we need.
4.1: At the very least I think the textboxes should each occupy a separate line
to make it simpler to look at!
5: "Author" field -- is this stored in cookie data? If not, it could easily be.
