I first mentioned these issues in Oct of 2007:

https://bugs.freedesktop.org/show_bug.cgi?id=12629 and have tried to  
catalog occasions where I ran into it.

I suppose I could add a specific bug for the Ban button being too  
close to other buttons, not having a warning and being totally  
undocumented or understood.  Then we can collect examples of it for a  
year and a half and be back where we are now.  Sorry, couldn't resist.
