Fix for glXCreate{Pixmap,Window}

I don't have write access to Mesa cvs, so I can't commit them. The
attached Mesa patch is what I committed to xorg.

Relative to the first patch, there are these changes:

   - i is not left uninitialized
   - It only allocates 8 * i extra bytes, not 20 + 8 * i
   - attrib_list is also allowed to be NULL for CreatePbuffer
   - The attributes are copied to data.

> is now the "authoritative" source for libGL.so.

By the next major release Xorg shoold just have Mesa as a dependency
instead of shipping its own copy.
