I have had *hell* with the i915gm chip: Here's what I did to fix things:
1. Disable all accelaration (Option NoAccel)
2. Always put "CRT" on pipe A (e.g. MonitorLayout "CRT,LFP")
3. Use the version from 0.7.1 or later (there's a bug in the i810
driver in 0.7.0 which was fixed in the CVS and the 0.7.1 branch).
Now everything works well for me (provided I connect my second monitor
at boot time).
