Moving monitor size information from mode to output record

Eric poked me today as he was implementing a piece of RandR support
about the fact that the proposed 1.2 version of RandR includes physical
size information in each mode. This prevents clone mode from working
between most monitors, which is clearly undesirable. In fact, there
really isn't a good reason to include the physical size information
there in any case.
I propose (and have already implemented) moving the physical size
information to the output. Of course, if switching modes changes the
physical size of the image, the driver can update the size and an event
will be delivered, so we don't lose any functionality, we just avoid
making modes accidentally different.
Unless I hear cogent objections, I'll take this as the right course.
