[Telepathy] stream-engine 0.4.0 released

Stream-engine 0.4.0 has been released and branched on a stable 0.4
branch which will not contain any exciting refactoring or new features,
and will track spec 0.16.x. This release contains various fixes for
improved robustness and clean shutdowns etc.
