The capital letter signalling and punctuation reading modes must be
defined by other means.  The say-as element is apparently intended for
special speaking of short pieces of text and can't contain subelements,
while the two modes usually apply globally.

Considering these facts, I don't know about a suitable way to express
the capital letter signalling and punctuation reading modes in SSML.
Any ideas?
