Oh, this looks like it is just the poppler bug I had originally seen
when I first created this test case. (I haven't actually verified that
the contents of the PDF file are correct according to the PDF
reference, but the rendering acroread gives matches what the test
program produces with the image backend, so the PDF file is at least
defacto correct).

I think Jeff had already tracked this poppler bug down a bit, but I
now actually reported it here:
