    mssdemux: also consider /manifest

    The spec says /Manifest, but /manifest is easily found on some
    services, so handle that, too.

This commit fixes the Manifest / manifest issue. For the CSM files support, it
seems to me (from a quick reading on the internet) that it is more of a
playlist than a media itself. Do you have any other stream examples? Is there
any open info online other than
http://msdn.microsoft.com/en-us/library/ff402314%28v=vs.95%29.aspx ?

I'm closing this bug as the main issue has been fixed. Would you mind opening a
new one to discuss the composite manifests implementation in case you are
interested in them?
