Merged the 2 patches, only missing the timestamp fixes now.

    qtdemux: store the moof-offset also for push mode

    It will be used in some cases for getting the correct offsets
    from trun atoms.

    qtdemux: handle default-base-is-moof flag

    Handle the flag from the tfhd that signals the base offset to
    start from the moof atom
